Please fill out and submit the follow form for take home quiz credit. All take home quizzes must be submitted electronically by the time of the exam to which the quiz relates. If you have trouble submitting electronically, please bring a hard copy (printout) of the take home quiz with you to the exam, and hand it in. You will need to submit the quiz electronically however to receive full credit. Your hardcopy is your proof that you completed the assignment on time.
Each question you submit must be in multiple choice format, include 5 possible answers (a,b,c,d,e), and indicate the correct answer.
Take home quizzes are due one lecture meeting date prior to the exam (i.e. if the exam is on Thursday, the quiz is due Tuesday). Please check to make sure your credit for the quiz is posted 24 hours prior to the exam. If it is not, please 1) email me, and 2) bring a hard copy (printout) of the take home quiz with you to the exam. NO LATE QUIZZES WILL BE ACCEPTED FOR ANY REASON.
